Insurance and Billing
In order to offer the highest level of personalized care, Dr. McClendon does not accept insurance. Minding Up is an “out of network” provider and can provide you with a superbill that you may submit to your insurance for reimbursement. A superbill includes the codes and charges for each service provided that your insurance company can use for reimbursement.

Cancellation Policy
We understand life circumstances may arise and require you to cancel or reschedule your appointment. However, we request you provide us with at least 24 hours’ notice if you need to reschedule or cancel your appointment. If you do not provide a 24 hours’ notice, you will be charged for the full appointment.
If you arrive more than 10 minutes after your scheduled appointment time, it will be considered a missed appointment. You will be charged for the cost of the full appointment and need to reschedule. We know emergencies happen, and there will be a one-time fee waive for a missed or late cancel appointment.
All fees and charges must be paid before scheduling your next appointment.
Frequent cancellations and no shows may result in being discharged from this practice.
